Many pediatric practices will not care for kids whose parents want to refuse some or all vaccines, or even to spread them out. This is outright discrimination.
There is an effort afoot in North Carolina to address this problem. This situation cries out for a legislative remedy. Here is some video from Joe Ladapo in Florida speaking to the issue:
